Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client's specific needs and goals. Outpatient programs provide individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

This is a union eligible position. Prototypes Outpatient (Pomona) is a Certified Community Behavioral Health Clinic (CCBHC), a grant funded program. The CCBHC model of care focuses on a client's whole person health, incorporating behavioral health and physical health care needs. The CCBHC program allows us to enhance the established behavioral health services our agency has been providing for decades and connect clients with community partners through care coordination partnerships in the area. This is a grant funded position through SAMHSA contract (H79SM086855) funded through 09/30/2026.

Job Summary

The CCBHC Intake Specialist is a SUD Clinical Therapist (LPHA) for the SAMHSA Certified Community Behavioral Health Center Improvement & Advancement (CCBHC-IA) grant responsible for conducting clinical assessments, diagnoses, and administering quality measures as required by the grant funder. Additional clinical tasks such as treatment planning, crisis management/intervention, and discharge planning may also be required. The LPHA works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcomes for the client, while utilizing different modalities including individual, group and family therapy. The LPHA is also responsible for reviewing, analyzing, and evaluating data for continuous quality improvement efforts in order to support CCBHC-IA objectives and enhance quality of services. Services may be provided in office, or in the field, based on client and program need. The position will provide both direct service and work to improve processes in the program. The LPHA is a professional who maintains a current BBS license or registration.

Key Responsibilities

• Conducts clinical assessments, diagnoses, and administers quality measures during outpatient intakes (baseline).
• Develops client-centered treatment plans, monitors treatment progress, and updates the treatment plan as needed.
• Provides crisis assessment and interventions as needed.
• Seeks clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstance.
• Participates in the 24-hour, 7-day per week after-hours coverage on rotation with other Therapists/LPHAs.
• Maintains compliance with due dates for all clinical documentation including progress notes, assessment and quality measures, treatment plan, treatment plan updates, re-assessments, discharges, outcome measures, and other clinical documents.
• Completes data management and Quality Improvement reports within a timely manner.
• Maintains full compliance with BBS requirements for registration and licensure.

Position Requirements

Education Requirements:

• Master's degree in Social Work, Marriage and Family Therapy or Clinical Counseling; and Registration in good standing with the BBS
• Highly preferred for the candidate to be licensed as LCSW, LMFT, or LPCC and in good standing through the Board of Behavioral Sciences (BBS)
• Highly desirable: Certification or Registration as a Substance Abuse Counselor

Experience:

• Preferable a minimum of two years' experience in DMH or SAPC services, treating clients with co-occurring mental health and substance use disorders
• Experience working with clients experiencing acute withdrawal from substances
• Knowledge of and experience with providing trauma informed services
• Experience delivering evidence-based practices
• Strong computer skills, including Outlook, Excel, Word and PowerPoint

Background Clearance:

• Must complete a background check and livescan.
• Must not be on parole or probation for a minimum of two years

Knowledge:

• Knowledge issues of related to substance abuse, mental health, and criminal background
• Culturally competent and able to work with a diverse population
• Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
• Bilingual- some positions may require a second language that reflects the client population
