Lead Care Manager: Whole-Person Care & Advocacy

Penny-Lane-Centers is seeking a Lead Care Manager in Los Angeles, California. This role involves direct service provision and acting as the primary contact for clients referred through the Managed Care plan. The Lead Care Manager coordinates care with various providers and assists clients in accessing community resources.
