Clinical ManagerThe Clinical Manager will primarily work in office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines and communication with referral sources. Coordinate patient admission with other members of the care team. Must have strong case management experience and working knowledge of Medicare. Vero Beach, FL (Martin/St. Lucie/Indian River Counties).Perform case management, oversight and coordination of care for all skilled disciplines.Close communication with social workersCoordinate patient admission with other members of the health care team.Provide nursing care and treatment of home care patients in accordance with the plan of treatment and approved authorizations.
