Description

• Assess patients in both inpatient and outpatient environments and tailor unique treatment plans based on individual needs
• Administer therapeutic interventions aimed at improving mobility, strength, balance, and overall function
• Work collaboratively with a diverse team of nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes
• Guide patients and their families on therapeutic goals, discharge strategies, and home exercise regimens
• Ensure thorough documentation of evaluations, treatments, and patient progress in line with institutional and regulatory standards
• Contribute to care team conferences and interdisciplinary meetings to foster coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specializations (such as neurological or geriatric) are advantageous but not mandatory

Skills
