RN Coordinator- Providence Home Services- Oregon
Schedule: Full Time/Days
RN Coordinator role consists of the planning and coordination of new referrals. The position will also provide phone triage of existing patients. The majority of the responsibilities will be carried out onsite in office. May provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor. These services are provided in the states of Oregon and/or southwest Washington.

Required Qualifications:
Upon hire: Oregon Registered Nurse License or
Upon hire: Washington Registered Nurse License.
Within 30 days of hire: National Provider BLS - American Heart Association.
Upon hire: Must be licensed in states wherever care is delivered.
3 years Home Infusion nursing experience.
Preferred Qualifications:
Bachelor's Degree in Nursing.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
