Contract Length: 13 weeks
Guaranteed Hours: 40 hours per week

Requirements
- 2 years of Physical Therapy experience
- HI state Physical Therapy license
- Bachelor's degree required; Master's degree preferred
- 1 year of home health experience
- Experience with Homecare Home Base and OASIS required
- Certifications: CPR, BLS (AHA)

Roles & Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical, social, and mental functions.
- Serve as co-leader in an interdisciplinary approach to patient care.
- Complete evaluations to meet quality standards and address assessment areas.
- Use appropriate clinical techniques to establish individualized treatment plans.
- Provide therapy interventions following established treatment protocols.
- Establish individualized care and discharge plans, ensuring communication with appropriate parties.
- Complete accurate clinical documentation within established deadlines.
- Supervise assigned assistants.

Compliance / Notes
- Must have own transportation, valid driver's license, and current auto insurance.
- Mileage reimbursement at $0.43/mile.
- Orientation and licensing requirements apply.
