Field-Based LTSS Case Manager & Care Coordinator

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) to manage a designated caseload in specialized programs. You will collaborate with individuals to lead the Person Centered Planning process, documenting their preferences, needs, and goals. Develop PCSPs and backup plans, while coordinating with Medical Directors to create an integrated care plan. Engage the individual’s support network, oversee physical and behavioral health management, and ensure adherence to state and #J-18808-Ljbffr Elevance Health
