Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility (SNF)

Compensation: $115,000 - $155,000 Annual Salary

Position Description

You will serve as the primary care and rounding provider inside one skilled nursing facility in Linwood, NJ, managing a consistent panel of residents rather than traveling between buildings. This is a full time, in person position, Monday through Friday, with no nights, no weekends and no on-call. The building cares for a higher acuity population that includes ventilator dependent residents, so you must be comfortable managing vent patients as part of your daily panel. You are employed by our provider network rather than the facility, so you do not report to the DON or the administrator, though you take part in patient care meetings.

Qualifications:

• Active New Jersey NP or PA license and DEA registration
• Must be comfortable treating ventilator dependent patients
• Experience in skilled nursing, long term care or geriatric primary care preferred
• Comfortable managing a full resident panel independently

Responsibilities:

• Provide primary care and rounding for residents in a skilled nursing facility
• Manage chronic conditions, acute changes in condition and transitions of care
• Manage the respiratory and ventilator dependent population alongside the clinical team
• Order and interpret labs and diagnostics and adjust treatment plans
• Document in the facility EMR and participate in patient care meetings
