Our nurses care for patients with a wide range of needs and conditions, including: Wound care and wound vac management Diabetes education and management Feeding tubes and other specialized treatments Chronic disease management Medication teaching and compliance Post-acute and complex medical patients Comprehensive patient and caregiver education You’ll serve as both a skilled clinician and case manager, coordinating care and helping patients achieve the best possible outcomes at home. Role Overview As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. What You Will Do Provide direct patient care in the home, applying nursing principles to assess and meet patient needs. Coordinate with allied health professionals to ensure integrated care delivery. Document patient care electronically using computer systems. Communicate effectively with patients, families, and healthcare personnel. What You Will Need Required Qualifications Graduation from an accredited Registered Nurse program. Current licensure as a Registered Nurse in the State of Michigan. Ability to effectively communicate with patients, families, and healthcare personnel. Knowledge of computer use for electronic documentation. Current driver’s license, satisfactory driving record, and reliable transportation. Ability to lift, carry, transport, and move clients as needed. Preferred Qualifications Minimum of nine months to one year of clinical nursing experience.
