Clinical Team Support – Home HealthLocation: Broward, FLSchedule: Monday–Friday | Full-Time | In-OfficeJoin Our Growing Home Health Team!We are seeking a highly organized and motivated Clinical Team Support to join our fast-paced home health agency. This position plays a vital role in coordinating patient visits, supporting our clinical staff, and ensuring seamless communication between clinicians, patients, and the office.If you thrive in a detail-oriented environment, enjoy problem-solving, and have excellent communication skills, we'd love to hear from you!ResponsibilitiesCoordinate and schedule patient visits for nurses and therapists.Maintain clinician schedules while optimizing productivity and geographic efficiency.Communicate with patients regarding appointment times and schedule changes.Collaborate with Clinical Managers and field staff to ensure continuity of care.Monitor daily schedules and make real-time adjustments as needed.Verify visit completion and follow up on missed or rescheduled visits.Assist with physician orders, referrals, and coordination of care as needed.Maintain accurate documentation within the electronic medical record (EMR).Answer incoming calls and provide exceptional customer service to patients, referral sources, and clinicians.Support the clinical team with administrative tasks and other duties as assigned.Previous scheduling experience in a healthcare setting.Home Health experience strongly preferred.Experience with WellSky/Kinnser or other EMR systems is a plus.Strong organizational and multitasking abilities.Excellent verbal and written communication skills.Ability to work efficiently in a fast-paced environment.Positive attitude with a strong attention to detail and customer service mindset.Why Senior Care Solutions?At Senior Care Solutions, our mission is to provide compassionate, high-quality care that allows patients to safely remain in the comfort of their homes.
