Requirements / Qualifications

• Certificate Copy (Certificate of proficiency in occupational therapy issued by the American Occupational Therapist Association)
• Copy of Transcript (Copy of Bachelor's Degree in Occupational Therapy from an accredited college or university )
• Letter of Introduction
• Letter(s) of Recommendation (Three letters of recommendation required, current letters preferred)
• Other (Copy of valid license to practice Occupational Therapy)
• Resume
