POSITION PURPOSE AND OBJECTIVES The purpose of this position is to help consumers with mental illness obtain and maintain competitive employment integrated in the community. This position will coordinate closely with the primary provider of service and, in some cases, provide services to consumers with physical disabilities or other mental disabilities. The role conducts at least 50% of activities in the community (e.g., job sites, client homes, or other community sites). Essential Job Functions Meet with consumers to provide intake and orientation to vocational services, gathering information about strengths, needs, preferences, desires, and abilities regarding meaningful activity. Assist consumers in developing opportunities for community involvement and meaningful activity. Advocate for consumers to employers who can include or exclude the consumer from the selected activity. Coordinate services with family members, significant others, or other agencies as appropriate. Work closely with the primary provider of service to ensure continuity of care, reporting changes in mental health for appropriate treatment decisions. Provide “follow‑along” services to consumers involved in meaningful activity whose symptoms may impact their ability to maintain the activity, including contact with supervisors, symptom monitoring, problem solving, and access to community resources. Provide on‑site consultation, observation, or assistance when the consumer is at risk of losing meaningful activity due to mental illness. Assist consumers in retaining benefits or making provisions for health benefits when the activity involves substantial earnings. Close cases in a timely fashion and complete all required documentation for the Ohio Department of Behavioral Health and Riveon Mental Health and Recovery, including writing and updating Individual Service Plans and documenting all contacts within one business day. Perform all other tasks as assigned. Knowledge, Skills, and Abilities Required Working knowledge of Riveon Mental Health and Recovery policy and procedures, with sensitivity and adherence to client rights, confidentiality, and health and safety issues. Demonstrated sensitivity to different cultures. Working Conditions Work occurs in settings including offices, client homes, various community locations, and businesses, requiring navigation of diverse facilities and weather conditions. Ability to walk, climb stairs, navigate changing surfaces (wet, slippery, icy), and work near electrical appliances, open flames, and areas with poor air quality. May need to lift and/or carry up to 10 pounds and interact with clients in emergency situations to prevent harm. Clear communication in verbal and written form. Extensive computer work requiring ability to sit for extended periods and operate required equipment. Requirements / Qualifications Bachelor’s degree in Social Work, Counseling, or related field (required). An associate’s degree or junior standing in a related field with at least 3 years of full‑time experience working with individuals with severe and persistent mental illness may be considered in lieu of a bachelor’s degree. Exceptional oral and written communication skills. Basic computer skills, including familiarity with Windows, Microsoft Office, and EMR. Professional business conduct. Understanding and advocacy for consumer rights, confidentiality, and health and safety issues. Cultural sensitivity. Physical ability to move safely in various work situations. Adequate hearing and speech for conversation. Spanish and English bilingual abilities are an advantage, though not required. Valid Ohio Driver’s license with fewer than 6 points in a 36‑month period. Possession of a vehicle with proof of insurance and willingness to transport consumers. Favorable references and/or evaluations. #J-18808-Ljbffr
