Job Title: RN Profession: Registered Nurse Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Not specified Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before the start date Must-Have: - Two (2) years of experience in a home setting within the last ten (10) years, including conducting comprehensive assessments - Experience working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care - Ability to conduct assessments similar to the UAS - Experience in primary care for homebound individuals - Experience in care management for individuals receiving home and community-based services - Home care experience caring for a minimum of two patients per week Description: Provide initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms related to the case. Enter evaluation data into electronic forms and transmit as required. Document any concerns or conflicting information surfaced during evaluations. Emphasize continuity of care to minimize fragmentation and gaps in treatment plans. Discuss healthcare options and supports needed with consumers. Initiate communication with eligible consumers for enrollment into managed care. Enhance communication and collaboration with interdisciplinary care team members. Collect quality review data and document case notes accurately. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and payor requirements. Meet all standards established for the position as outlined in performance criteria. Perform other duties as assigned by the Regional Nurse Manager or project management.
