This is an on-site, in person role where the Registered Nurse RN PRN supports hospice care for older adults in their home, through evening, overnight, weekend, and holiday on-call rotations. Reporting to the Director of Nursing, this role provides compassionate clinical care, staffing coverage, admission support, and urgent patient assessments as needed.

Responsibilities

• Assess hospice patients and respond to urgent clinical needs.
• Develop and update individualized care plans.
• Administer medications safely and effectively.
• Monitor patient condition and provide appropriate clinical interventions.
• Educate patients and families regarding care and treatment.
• Coordinate care with patients, families, and the hospice team.
• Maintain accurate clinical documentation and electronic health records.
• Provide staffing coverage, admission support, and evening, overnight, weekend, and holiday on-call coverage.

Required Qualifications

• Associate Degree in Nursing.
• Active Registered Nurse license.
• Basic Life Support certification.

Preferred Qualifications

• More than one year of hospice nursing experience.
• Experience with patient assessment, medication administration, electronic health records, care planning, wound care, and clinical documentation.
• Strong critical thinking, communication, and time-management skills.
