Permanent Medical Director Geriatric Medicine

Rate: $231,000 $280,000 Annual Salary Depending on Experience
Shift: Full-Time Day Shift

End Date: Permanent Opportunity

Position Details:
Patient Population: Frail & Elderly Patients
Setting: PACE Program / Community-Based Geriatric Care
Responsibilities: Providing medical leadership, overseeing participant care, supervising PCPs & APPs, managing QAPI initiatives, conducting assessments, chronic disease management, interdisciplinary collaboration, and comprehensive geriatric care coordination
Required Procedures: Geriatric Assessments, Chronic Disease Management & PACE Care Oversight
EMR System: Facility EMR System
Support Staff: PCPs, APPs, Nursing Team & Interdisciplinary PACE Staff
Days Per Month: Full-Time Coverage
On-Call/Weekends: No Weekends On-Call Rotation As Required

Submission Requirements:

Board Certified in Internal Medicine or Family Medicine Required
Fellowship Training or CAQ in Geriatric Medicine Preferred
Indiana License or Willingness to Obtain Required
CDS, DEA & BLS Required by Start Date
Minimum 1 Year Experience with Frail/Elderly Population Required
Previous Supervisory Experience Required
