Description

• Conduct thorough evaluations of patients in either inpatient or outpatient settings and develop personalized treatment plans
• Implement physical therapy interventions that enhance mobility, build strength, improve balance and overall function
• Collaborate with a diverse team of nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and patient progress in alignment with facility policies and regulations
• Engage in multidisciplinary care rounds and conferences to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferable
• Specialty certifications (such as neurological or geriatric specialties) are beneficial but not mandatory

Skills

• Prior experience is strongly preferred
• Capability to treat patients with complex medical conditions and mobility challenges
• Excellent communication and teamwork abilities
• Proficient in digital charting and clinical documentation
