Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafted individualized treatment plans tailored to their unique needs
• Administer an array of physical therapy interventions, focusing on enhancing mobility, strength, balance, and overall functional performance
• Work collaboratively with a diverse team of nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge planning, and home-based exercise programs
• Ensure comprehensive documentation of evaluations, treatments, and patient progress adhering to facility standards and regulatory requirements
• Engage in interdisciplinary conferences and care team rounds to facilitate coordinated and holistic care

Education

• Doctorate in Physical Therapy (DPT) or equivalent credentials from an accredited educational program

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (for example, in neurological or geriatric therapy) are beneficial but not mandatory

• Prior experience in a clinical setting is strongly preferred
