We are seeking a licensed and motivated Physical Therapist (PT) to evaluate residents, develop individualized treatment plans, and deliver high-quality rehabilitative services in a skilled nursing setting.

Position Overview

• Position: Physical Therapist (PT)

• Status: Full-Time

• Sign-On Bonus: $5,000

• Setting: Skilled Nursing and Rehabilitation

Part-time and PRN opportunities may also be available.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans based on physician orders and residents’ needs.

• Provide physical therapy treatments and adjust interventions as needed to help residents achieve functional goals.

• Supervise Physical Therapist Assistants, rehabilitation technicians, and therapy students as required.

• Maintain accurate and timely documentation in accordance with clinical and regulatory standards.

• Collaborate with interdisciplinary team members, including nurses, physicians, and occupational therapists.

• Educate residents, families, and staff on therapeutic techniques, mobility, and exercise programs.

• Participate in discharge planning and facility quality improvement initiatives.

• Travel to or provide coverage at nearby facilities as needed.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current state licensure as a Physical Therapist or eligibility to obtain licensure.

• Experience in a skilled nursing or long-term care setting is preferred but not required.

• Strong clinical judgment, communication, and interpersonal skills.

• Ability to assist residents with mobility and perform the physical demands of the role.

Work Environment

This is an active, hands-on clinical role that involves standing, walking, bending, and assisting residents with mobility throughout the workday. The position also requires regular collaboration with residents, families, and multidisciplinary care teams.
