What You'll Do As an LPN at CCHS, you'll work closely with providers and other members of the clinical team to support safe, efficient, and coordinated patient care. Your responsibilities will include: Performing nursing assessments and obtaining vital signs, health histories, and other clinical informationPerforming routine tests and screenings and communicating findings or concerns to providersAdministering medications, treatments, injections, and immunizations as orderedAssisting providers with patient examinations and proceduresTriaging patient calls and visits within the LPN scope of practiceProviding patients and caregivers with education regarding medications, treatment plans, preventive care, and other healthcare needsCoordinating specialist referrals, testing, procedures, appointments, and prior authorizationsSupporting patients with complex healthcare needs through care coordination and care management activitiesMaintaining accurate and complete clinical documentationReviewing immunization status and administering and documenting immunizationsCollaborating with providers, clinical staff, patients, caregivers, and outside healthcare facilities to support continuity of careParticipating in Quality Improvement activities and supporting CCHS clinical standards and protocols What We're Looking For Degree or diploma in Practical Nursing from an accredited nursing programCurrent and valid Licensed Practical Nurse (LPN) license in the state of GeorgiaCurrent BLS/CPR certificationOne to three years of nursing experience in an outpatient or primary care setting preferredKnowledge of nursing assessment, medication administration, injections, immunizations, and other clinical procedures within the LPN scope of practiceStrong communication, organization, and patient education skillsAbility to recognize and communicate changes in a patient's condition and respond appropriatelyAbility to use an Electronic Health Record (EHR) systemAbility to work collaboratively as part of a healthcare teamBilingual in English and Spanish is a plus Why Join CCHS?
