Care Manager Social Worker - Oncology Services [Remote]

This role provides comprehensive psychosocial and care-management support to oncology patients and their families across the healthcare continuum.
You will partner closely with physicians, nurses, care teams, and community resources to identify and address barriers to care.
The position combines psychosocial assessment, counseling, patient education, resource coordination, and care navigation.
You will help patients manage complex social, emotional, and practical challenges while promoting self-management and improved outcomes.

This is a full-time, Monday-to-Friday opportunity with a focus on oncology services and coordinated patient care.

Accountabilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans addressing identified patient and family needs.
• Identify barriers to care, including socioeconomic challenges, unmet social needs, impaired coping, limited support systems, behavioral health concerns, and access issues.
• Coordinate care with physicians, nurses, medical providers, patients, families, community organizations, and other healthcare professionals across the care continuum.
• Provide counseling, education, patient advocacy, referrals, and navigation support to help patients access appropriate medical, social, and community resources.
• Monitor, evaluate, and reassess care-management plans, adjusting interventions as patient circumstances and needs evolve.
• Support transitions of care, discharge planning, risk stratification, and appropriate utilization of healthcare resources.
• Promote self-management strategies and help patients and families navigate complex healthcare systems while improving quality and continuity of care.
• Collaborate with interdisciplinary teams to address complex situations and develop practical solutions that support positive patient outcomes.
• Maintain appropriate documentation and follow applicable regulatory, privacy, compliance, and professional standards.
• Serve as a resource to patients, families, providers, and colleagues while maintaining a high standard of customer service and professional social work ethics.
• Contribute to care-management initiatives across ambulatory, inpatient, and community settings, including telephonic, office, and home-based interactions as applicable.

Requirements

• Bachelor’s or Master’s degree in Social Work from an accredited school of social work; a Master’s degree is preferred.
• Current Maryland social work licensure, with the ability to maintain licensure and complete required continuing education.
• LCSW certification/licensure is required.
• Approximately 3–5 years of healthcare and/or care-management experience.
• Care-management certification, such as Accredited Case Manager (ACM), is preferred.
• CPR/BLS certification is required, unless exempt under an approved full-time remote arrangement.
• Strong understanding of clinical and psychosocial aspects of patient care, healthcare navigation, and community resources.
• Excellent interpersonal, written, and verbal communication skills, with the ability to explain complex issues clearly and compassionately.
• Strong critical-thinking, organization, time-management, and problem-solving abilities, including the ability to manage multiple priorities in a fast-paced environment.
• Ability to work independently while collaborating effectively with physicians, clinical teams, patients, families, and community partners.
• Demonstrated ability to advocate for patients while balancing individual needs with care-management and healthcare-system objectives.
• Sound judgment and discretion when handling sensitive, confidential, or complex patient situations.
• Working knowledge of basic computer applications and healthcare documentation processes.
• Ability to adapt to changing priorities, take initiative, and contribute to new projects and process improvements.
• Ability to provide appropriate care and communication across the full patient age spectrum, from neonates through geriatric patients.
• Ability to perform light physical work, including occasional lifting of up to 20 pounds and periods of walking or standing.
