Position Summary Vitae Healthis seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes. Key Responsibilities Conduct comprehensive psychosocial assessments for residents upon admission and as needed Develop, implement, and monitor individualized treatment plans Provide individual, group, and/or family therapy sessions Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns Collaborate withthe Vitae clinical team, the patient’s attendingphysician,facilitynursing staff, social workers, and rehabilitation teams to coordinate care Participate in care plan meetings and interdisciplinary team discussions Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations Provide crisis intervention and behavioral management strategies when needed Educate staff and families on mental health conditions and effective communication strategies Ensure compliance with HIPAA and ethical standards of practice Master’sdegree in Psychology, Counseling, Social Work, or related field Active and unrestricted license inrespective state(e.g., LCSW, LPC, LMFT, or Licensed Psychologist) 2years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings Strong clinical assessment, diagnostic, and treatment planning skills Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders) Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling) Ability to work collaboratively within an interdisciplinary team Excellent communication and interpersonal skills with residents, families, and staff Strong organizational, time management, and documentation skills Familiarity with electronic health records (EHR) systems preferred Excellent earning potential with competitive base salary plus uncapped bonus opportunities or flexible fee-for-service options Consistent, reliable referral flow so you can focus on patient care,not building a caseload Comprehensive benefits package including Health, Vision, and Dental insurance Generous paid time off: 3 weeks PTO, 6 paid holidays, plus 1 dedicated day for education and professional development Full malpractice coverage including tail insurance,no out-of-pocket costs Retirement planning support with a 401(k) and company match Ongoing professional growth through CEU opportunities and paid licensure renewals Supportive, clinician-focused leadership team that prioritizes your success and well-being Up to a$3,000 referral bonus,get rewarded for bringing great clinicians to the team Equal Opportunity Statement Vitae Healthis an Equal Opportunity Employer and does not discriminate based on race, color, religion, sex, national origin, age, disability, or any other protected status. #J-18808-Ljbffr Vitae Health Systems
