Nurses who thrive here value close patient relationships, clinical excellence, and meaningful work.As a vital member of our home health team, you will:Provide skilled nursing care tailored to each client's unique needsMonitor health conditions and document changes in clinical statusAdminister medications and perform wound careDeliver G-tube, trach, or vent support when requiredEducate and guide clients and families to support ongoing health at homeCommunicate effectively with the care team to ensure continuity of careWe are looking for an LPN who is:Licensed as an LPN and in good standing with the Georgia Board of NursingExperienced in a clinical setting, with at least one year preferredCertified in CPRSkilled in EMR documentation, with strong organizational abilityConfident working independently in a home environmentCompassionate, client-focused, and a strong communicatorExperienced with G-tube, trach, or vent careLicensed to drive with reliable transportationWe prioritize both patient well-being and nurse satisfaction.
