Registered Nurse Case Management

Your future role at a glance Location: Tulsa, OK Facility: Ascension St. John's Medical Center Department/Specialty: Case Management Schedule: FT, Friday - Monday, 7:30am-6pm How you'll make an impact in this role Improve hospital efficiency and finances by reducing stays, streamlining patient flow, and optimizing resource use, which boosts revenue and minimizes waste. Enhance care transitions, lower readmissions, and connect patients with community resources, leading to better satisfaction and addressing social needs. Support clinical teams by bridging gaps, advocating for patients, and ensuring comprehensive, timely care, resulting in higher quality, lower costs, and better patient outcomes. What minimum qualifications you'll need Licensure / Certification / Registration: Registered Nurse obtained prior to hire date or job transfer date required. Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
