Case Manager - Med Only Team $53,728 - $69,847, MI Join our Med Service Team at Centra Wellness Network (CWN). As a member of the Behavioral Health Home team, you provide services and supports to adults diagnosed with serious and persistent mental illness, which may include co‑occurring substance use disorder, through implementation of the Health Homes Care Plan. You support individuals managing symptoms in support of health goals through outreach, community education, information counseling, social support, and advocacy. You manage all meds‑only cases, providing coordination, linking with PCP and others for transfer of medication services for Medications Only clients. The employee will complete other tasks as assigned by supervisor. Staff must possess computer, typing, grammar, punctuation, proofreading, transcription, phone proficiencies and excellent communication and customer relations skills to effectively interact with staff, customers, business partners and community stakeholders. Additionally, the employee must exercise independent decision‑making and work effectively in a team‑based environment. Provide services such as linking and coordination for clients within the Behavioral Health Home. Manage all meds only cases, providing coordination, linking with PCP and others for transfer of medication services for Medications Only clients. Provide education and support to all BHH clients. Obtain a Community Health Worker certification to assist with providing support to clients in Meds Only and BHH. Must have or be actively working toward the Community Health Worker Certification as required by MDHHS. Often will be assigned to complete other tasks as directed by supervisor. Requirements Bachelor’s degree in Social Work or a related field, as outlined by the State of MI Medicaid Provider Manual. Full Michigan licensure preferred or Michigan Limited License in field of practice considered. Applicant must be eligible to complete supervision standards to meet full licensure within the first two years of employment. Full Michigan Licensure will be expected for continued employment. Ability to obtain QMHP and/or QIDP appointment within the first year. Must have or be actively working toward the Community Health Worker Certification as required by MDHHS. At least 18 years of age. Valid Michigan driver’s license and good driving record. Ability to be certified in First Aid and CPR within 90 days of hire (agency available training).
