We are seeking a compassionate and dependable Host Home Provider to open your home and provide a supportive, family-like environment for a 25-year-old high-functioning male enrolled in the Home and Community-based Services (HCS) program.

This is a rewarding opportunity to positively impact someone’s life while receiving ongoing support from our agency.

Responsibilities

Provide a safe, stable, and welcoming home environment.
Assist with daily living skills as needed while encouraging independence.
Support community participation, recreational activities, and social engagement.
Provide transportation to appointments and activities as needed.
Communicate regularly with the individual’s support team.
Maintain required documentation in accordance with HCS guidelines.

Qualifications

Must be at least 18 years of age.
Must have a valid driver’s license, reliable transportation, and auto insurance.
Must pass required background checks.
Must be willing to complete HCS Host Home training (provided by the agency).
Previous caregiving experience is preferred but not required.
