About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in OASIS and HomeCare HomeBase EMR within the last 6 months - Open to traveling up to 75 miles from assigned care center - Willingness to participate in on-call rotations Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. Care is delivered based on agency policies and procedures through the competent application of the nursing process. Responsible for supervising and evaluating home health aides and licensed practical nurses. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity holds a different point value. Contractors will be assigned between 5-10 patients per day, equating to roughly 6-8 points. Types of cases include start of care, wound care, and follow-up visits. Weekly productivity expectations are between 25 and 30 points. Documentation must be completed within 24 hours of the appointment. All travelers must be proficient with the required EMR systems. The contractor will cover designated territories and must be comfortable with daily travel. No contract hours are guaranteed; clinicians must accept all patients on their schedule. Declining patients may result in cancellation of the contract.
