Registered Nurse - Hospice

Job Description

Job Summary:
Provides coordinated home nursing care for hospice patients according to physician orders and agency policies. Oversees and evaluates home health aides and licensed practical nurses/licensed vocational nurses to ensure quality patient care.

Responsibilities:

• Coordinate and deliver in-home hospice nursing services based on established care plans.
• Supervise and evaluate home health aides and nursing staff to maintain compliance and quality standards.
• Perform comprehensive assessments, develop care plans, and document patient progress.
• Travel up to 75 miles for patient visits, ensuring timely and efficient delivery of care.
• Complete all documentation within 24 hours of patient visits to ensure accurate billing and patient follow-up.
• Maintain proficiency in HomeCare HomeBase (HCHB) and Oasis for appropriate documentation and charting.

Qualifications:

• Current RN license in the state of practice with no disciplinary actions.
• Minimum 1 year of recent adult home health or hospice experience.
• Valid driver's license, reliable transportation, and agency-required auto insurance.
• Proficiency in HomeCare HomeBase (HCHB); Oasis experience required for RNs.
• Ability to travel up to 75 miles and accept scheduled patient visits regardless of distance.
