Job Summary:
The organization is seeking a Physical Therapist specializing in Home Health in the specified region. The role involves delivering patient-centered care within community health settings, assessing patient conditions, implementing treatment plans, and providing education and support to patients and their families. The position requires adherence to organizational policies and standards, utilizing electronic documentation and interdisciplinary communication to ensure quality care.
Job Details:
- Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Valid state license as a Physical Therapist, minimum of 1 year of experience in home health, and 1 year of case management experience. Experience in hospice, private care, or solely as a case manager without home health agency experience does not meet the qualifications. Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks. No experience minimum is listed apart from the specified years.
- Additional Information: Responsibilities include performing admission processes and case management duties, documenting electronically and communicating effectively with interdisciplinary teams. The start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled start will be necessary.
