RN Care Manager

Coordinates and manages the care of patients with chronic or complex conditions Works collaboratively with physicians, interdisciplinary teams, individual patients, and families Performs comprehensive care continuum process including assessment, planning, implementation, coordination, monitoring, and evaluation Advocates for, educates, and coaches patients, families, and caregivers on treatment options Supports goal-setting and development of self-care skills Facilitates communication and coordination among members of the healthcare team Requirements Current Registered Nurse (RN) license in state of practice Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required) Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes Demonstrated experience in care management Certified Case Manager (CCM) or other relevant certification preferred Two (2) years of experience in nursing, preferably in primary care or ambulatory care settings One (1) year of experience in care management Experience in chronic disease management Knowledge of value-based care models and principles Core Competencies Demonstrates expertise in care management and chronic disease management, with a strong focus on patient advocacy, education, and interdisciplinary collaboration.
