Job ID: View phone number on click.appcast.io Job Overview The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Responsibilities include medication administration, documentation, SHU conduct, resident care, and adherence to Sunrise Senior Living Mission and regulatory standards. Responsibilities Administer medication and treatments safely, following the Six Rights of Medication Pass. Review, endorse, and document all medication administration in the electronic health record. Maintain and restock medication carts, medication and treatment rooms, and ensure all items are clean and up to date. Report medication concerns, unavailable medication, and abuse or violations to the Resident Care Director (RCD) or Wellness Nurse. Assist in checking medication regardless of packaging system and assess residents for as‑needed medication requirements. Count narcotics with another Medication Care Manager or Lead Care Manager each shift and report discrepancies. Maintain confidentiality of all resident information, including medication records. Participate in daily Cross‑Over meetings, Service and Health Updates, and Individualized Service Plan (ISP) development. Serve as a liaison for risk management and general safety, reporting incidents, hazardous conditions, and occupational exposures. Practice infection control, OSHA safety procedures, and proper handling of bio‑hazardous waste. Offer support and maintain residents’ independence, dignity, and physical safety aligned with Sunrise Principles of Service. Reference the Six Rights and standard care precautions, including cleanliness and hygiene, throughout patient interactions. Contribute to team engagement, training attendance, and may supervise other Medication Care Managers as assigned. Qualifications LPN, LVN, or state/province specific licensed nurse credential. Minimum 18 years of age. Previous experience working with seniors preferred. Commitment to serve and care for seniors. Must be able to perform all essential duties satisfactorily. Problem solving, judgment, and risk management aptitude. Certifications & Training Maintain CPR and First Aid certifications as required. Maintain all state/provincial certifications, ongoing training, and re‑education required by Sunrise and governing bodies.
