Responsibilities
- Provide and document nursing care for patients at home, including assessment, monitoring, medications, and symptom management
- Implement and adjust care plans to promote, maintain, or restore health and support rehabilitation in the home setting
- Collaborate with interdisciplinary team members and caregivers to coordinate care and transitions, ensuring continuity and safety
- Exercise independent clinical judgment within established policies and protocols; maintain high standards of patient privacy and confidentiality
- Maintain accurate records, communicate clinical data effectively, and respond to changes in patient status with appropriate escalation
- Travel to patient homes using a personal vehicle; manage time and caseload with flexibility; participate in on-call duties as assigned
- When trained and allowed, provide infusion therapy and other advanced nursing interventions as appropriate

Requirements
- Licensed Practical Nurse license active in Massachusetts (Must be uploaded with profile)
- Massachusetts Temporary Nursing Service Agency License (Nursing Pool) required up front and must be uploaded with traveler profile
- One year of acute nursing care experience required; hospital-based experience preferred
- Experience in home health required; Certified Home Health with proficiency in OASIS highly preferred
- Infusion therapy experience preferred
- Active BLS or equivalent American Heart Association/Red Cross certification required
- Current driver's license and access to reliable transportation; traveler must have own vehicle; able to drive in inclement weather
- Demonstrated ability to work independently in home-based care and to manage a caseload with limited supervision

- Supervisory signed reference from within the past two years of employment [REQUIRED] [DOCUMENT]
- RTO requests must be noted in profile; requests after submission may not be approved
- All scheduling requests must be included in the profile at submission; blanket or late requests will not be accommodated
- Geographic restriction applies: candidates must reside beyond 50 miles from the work site
- Availability for on-call responsibilities as required
