Position Physical Therapist (PT) – Home Health – Signature Healthcare at Home (A Pennant Group Company) Location: Lincoln City, OR – Oregon Coast Position Summary As a Home Health Physical Therapist, you will provide personalized, one‑on‑one care to patients in their homes—helping them regain mobility, independence, and quality of life. This is a field‑based role offering flexibility, autonomy, and the opportunity to build meaningful patient relationships. Key Responsibilities Patient Care: Evaluate and treat patients recovering from injury, illness, or surgery. Develop and implement individualized plan of care. Improve patient mobility, strength, balance, and functional independence. Provide interventions focused on fall prevention and safety. Care Coordination: Collaborate with physicians, nurses, and interdisciplinary team members. Adjust treatment plans based on patient progress. Participate in case conferences and care planning. Patient & Caregiver Education: Teach patients and families exercises, safety strategies, and mobility techniques. Empower patients to manage their conditions at home. Documentation & Compliance: Complete accurate and timely clinical documentation (OASIS experience preferred). Follow all Medicare home health regulations and Oregon state requirements. Quality & Outcomes: Support improved patient outcomes and satisfaction. Reduce rehospitalizations and promote safe recovery at home. Qualifications Licensed Physical Therapist (PT) in Oregon (or eligible). Graduate of an accredited Physical Therapy program. Valid driver’s license and reliable transportation. Ability to work independently in a field‑based setting. Preferred: Prior home health experience. Familiarity with OASIS and Medicare documentation. Experience serving rural or geographically spread communities. Compensation & Benefits Salary Range: $55 – $65 hourly (DOE). Sign On Bonus: $10,000. Mileage reimbursement.
