In-Home Health - NP or PA (Part Time) - Madison County/ Syracuse, NY

Position Overview Part‑time Clinician (Nurse Practitioner or Physician Assistant) providing in‑home health evaluations—which include medication and medical history review, physical evaluation, and, when requested by a health plan, diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive workflow and are not required to prescribe medication or alter treatment regimens. Key Responsibilities Conduct in‑home visits, lasting about 45 minutes, to connect one‑on‑one with patients and answer health questions. Gather medication and medical history, perform a physical evaluation, and coordinate any additional diagnostic tests as directed by the health plan. Use the Signify Health clinical workflow to provide diagnostic and preventive services such as spirometry and diabetic eye exams. Collaborate with Signify Health colleagues and fulfill all specified role requirements. Travel locally and within licensed states as needed; conduct virtual visits when required. Maintain high standards of safety and quality while working in diverse home environments. Required Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Must be able to work in the private residences of health‑plan members. Ability to travel locally and within licensed states. Preferred Qualifications Multi‑state licenses. Prior experience in home or community‑based clinical settings. Comfortable performing duties in homes that may include pets, secondhand smoke or allergens. Location Targeted primary locations: Madison County / Syracuse, New York. Must be able to travel locally and within licensed states. Work Hours Anticipated weekly hours: 29.5 (part‑time). Pay Range Base hourly rate: $46.03 – $99.14. The actual offer will depend on experience, education, geography and other factors.
