El aprendiz de Trabajador de Salud Comunitaria es un miembro del equipo de salud comunitaria; un equipo interdisciplinario compuesto por Trabajadores de Salud Comunitaria, Clinicianos de Salud Conductual y Administradores de Atención Enfermera basados en la práctica. Trabaja en estrecha colaboración con los miembros del equipo principal para apoyar a pacientes que enfrentan problemas médicos, de salud conductual y/o de sustancias complejos, así como determinantes sociales de la salud, y que requieren una intervención más intensiva en el hogar y en la comunidad. El Trabajador de Salud Comunitaria desempeña un papel crítico en la evaluación de las necesidades de los pacientes, ofreciendo recursos comunitarios y referencias, proporcionando navegación, apoyo, coordinación de atención y gestión continua de casos para satisfacer esas necesidades. Visita a los pacientes en sus hogares y en las comunidades en las que viven, proporcionando enfoques culturalmente sensibles para la información sobre salud para mejorar la alfabetización en salud. El Trabajador de Salud Comunitaria facilita la toma de decisiones y la autogestión del paciente para ayudar a los pacientes a participar en su salud general y alcanzar sus objetivos de salud. Esta persona será responsable de rastrear actividades relacionadas con los pacientes, monitorear y documentar el progreso. Trabaja en colaboración con el equipo de atención primaria para promover la atención centrada en el paciente y participa activamente en reuniones del equipo multidisciplinario centradas en el paciente. El Trabajador de Salud Comunitaria tiene contacto frecuente con muchas agencias comunitarias en nombre de los pacientes atendidos, estableciendo redes y colaborando en la identificación de recursos para mejorar la salud general de la población. El desarrollo de un plan de atención al paciente incluye acciones diseñadas para mejorar el estado de salud del paciente y eliminar las barreras que les impiden acceder a atención primaria/especializada de alta calidad y oportuna. Aprovecha los informes de registros de EMR/enfermedades crónicas para priorizar el seguimiento de los pacientes. Identifica y utiliza enfoques culturalmente sensibles y recursos comunitarios. Documenta actividades y comunicaciones en el registro de pacientes según lo definido por los requisitos del programa. Proporciona capacitación a otros miembros del equipo o personal de la práctica según sea necesario. Actúa como enlace con planes de salud, hospitales, cuidados a largo plazo, especialistas en salud conductual y representantes de salud en el hogar. Asiste a la capacitación y sesiones colaborativas requeridas según lo programado. Realiza otras tareas relacionadas según se solicite y forma parte de varios comités de todo el sistema.
