Location: 1 Abrahms Boulevard, West Hartford, Connecticut, 06117 Physical Therapy Aide or COTA for Rehab Unit- Discharge planner. What You'll Do: You will play a pivotal role in ensuring the well-being and comfort of our residents. You will collaborate with healthcare professionals, and oversee the implementation of care plans.
