This role serves as a key link between referral sources and the Health Center, ensuring timely admissions, smooth resident transitions, and exceptional customer service throughout the admission process.### Responsibilities* Achieve occupancy and census goals.* Review referrals and clinical documentation to determine admission eligibility.* Conduct clinical assessments and coordinate admissions with internal teams.* Ensure smooth transitions between hospitals, skilled nursing, and other levels of care.* Triage hospitalized residents and coordinate appropriate placement or return to care.* Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, and other referral sources.* Conduct hospital visits, bedside assessments, rounds, and community outreach activities.* Track referrals, admissions, and professional contacts within CRM/EHR systems.* Collaborate with admissions, clinical, and operations teams on referral and census management.* Support outreach events, marketing initiatives, and community engagement efforts.* Assist with training and guidance of Admissions Coordinators.Posted2 days agoJob TypeFull-timeSalary$80,000 – $95,000 USDLocation* Lincolnshire, IL
