Physical Therapist (PT)

Full-Time | Skilled Nursing / Long-Term Care | Virginia, Minnesota

Pay Range: $40.00 - $50.00 / hour (Determined by experience, clinical specialty, and qualifications)

Position overview

Above All Talent Solutions is partnering with a rapidly growing therapy provider that delivers innovative rehab programs to Senior Living communities. This full-time Physical Therapist position may have potential to support nearby sister facilities, serving adult and geriatric residents in a post-acute settings. As a key member of an interdisciplinary rehab team, you will help residents improve mobility, strength, balance, and pain management to maximize safety, independence, and quality of life.

Responsibilities

• Evaluate, diagnose, and treat adult and geriatric residents with mobility, strength, balance, gait, and pain impairments in a SNF/LTC environment.
• Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, OT, SLP, and medical staff.
• Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards.
• Educate residents, families, and staff on therapeutic exercises, mobility aids, fall prevention, and home safety.
• Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.

Qualifications

• Current, unrestricted Physical Therapist license in the state of practice (or eligibility prior to start).
• Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred.
• Excellent communication, documentation, and collaboration skills; resident-centered, outcomes-focused approach.
