Minimum Requirements
Bachelors, Masters, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
Active Indiana PT license with no restrictions required to start the assignment.
Minimum one year of physical therapy experience required.
Basic Life Support (BLS) certification required (AHA or ARC accepted).
Home health experience preferred.
Experience with HCHB and OASIS preferred.

Assignment Details
Facility Type: Home Health.
Shift Name: Days.
Shift Details: Days 0800-1600. Monday–Friday schedule reported as 8:00 AM–4:30 PM for typical coverage.
Weekly Hours: 40 hour guarantee indicated.
On-call/Weekend Requirement: One weekend per month required.
Coverage Area: Multiple counties including Clark, Floyd, Harrison, Jefferson, Scott, Washington, and Crawford (travel between patient homes expected). Mileage reimbursed between visits from first to last patient.
Primary responsibilities: Regular home visits, start-of-care, resumption-of-care, and recertification visits; documentation in HCHB and completion of OASIS assessments.
Education: Minimum Bachelors Degree required.
