POSITION SUMMARY RN Case Manager

POSITION DUTIES Skilled Nursing review

Skills, key job functions include assessment of identified members, development of a safe discharge plan from acute inpatient, skilled nursing, LTAC and Inpatient Rehab facilities.

MINIMUM REQUIRED QUALIFICATIONS license yes;

• Certificates required: CCM preferred but not required; experience with skilled nursing facilities and inpatient acute care

PREFERRED QUALIFICATIONS Kaiser experience

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK 

• Shift: 8a-5p
• Shift notes (weekends): Every other weekend preferred (negotiable); 40 hours
