Job Summary:

The Physical Therapist provides specialized rehabilitative services to patients, assisting them in restoring movement and function following injury or illness. They evaluate patient needs and develop personalized treatment plans to promote optimal recovery.

Responsibilities:

  Assess patients physical condition and establish detailed treatment goals.

  Design and implement therapeutic exercises and modalities to improve mobility and strength.

  Monitor patient progress and adjust treatment plans accordingly.

  Educate patients and caregivers on techniques to enhance recovery and prevent recurrence.

  Document patient progress and communicate effectively with healthcare team members.
  
  

Qualifications:

• 
  Hold a valid state license and relevant certification as a Physical Therapist.

  Demonstrate experience working in SNF or long-term care settings.
