This setting offers meaningful, relationship-based care where you can see the direct impact of your work on patients' daily lives.Typical responsibilities include completing initial and follow-up assessments, developing and updating individualized plans of care, administering medications, monitoring vital signs, performing wound care and other skilled nursing procedures, and teaching patients and caregivers about disease management, medication regimens, and safety in the home. You will communicate regularly with physicians, therapists, social workers, and other team members to ensure coordinated, patient-centered care.A typical day in this role includes visiting multiple patients in their homes, documenting care and progress using an electronic medical record system, and adjusting care plans based on clinical findings. You will organize your schedule to balance skilled visits, follow-up checks, and coordination tasks, while maintaining timely and accurate documentation. Caseloads and visit numbers are managed to support safe and effective care, with a focus on quality outcomes and patient satisfaction rather than high-volume throughput.This position is ideal for an RN who enjoys autonomy in practice, critical thinking, and building strong connections with patients in their own environment.
