Job Description

Up to a $10,000 Sign on Bonus Available

Schedule: Full-Time/Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation.

Location: Barington, Schaumburg, Batavia, Carol Stream, Hoffman Estates, Itasca, Medinah, Roselle, St Charles, and Steamwood Area with travel to LaGrange Hospital for meetings/Orientation etc.

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time, Part Time and PRN available

Company provided phone and laptop/iPad

• Implements and monitors patient care plans to ensure effective treatment.
• Monitors, records, and communicates patient condition as appropriate.
• Serves as the primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders accurately and promptly.
• Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.
• Administers medications and solutions as required.
• Documents all phases of patient status during care.
• Participates in the transfer of patient information to ensure continued quality care.
• Leads and mentors staff to ensure optimal performance and competency in care delivery.
• Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.
• Develops and maintains positive working relationships within the practice and other departments.
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Work Experience:

• 1+ acute care clinical experience [Preferred]
• 1+ home health experience [Preferred]
