Location: Lake Wales, FL 33898
Setting: Patient's Home (Home Health)
Pay: $110 per evaluation

Responsibilities include but are not limited to:

• Evaluates and treats patients in a Home Setting
• Communicates with families, physicians, and other health care professions
• Maintains daily, thorough documentation of services in the EMR
• Having your own equipment including smart devices and blood pressure monitor.

Qualifications:

• Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy ,
• Education and/or Experience: New graduate applicants welcome + Experienced Therapists
• Actively Licensed as a FL Physical Therapist.

Job Type: PRN (As Needed)

PRN (from the Latin pro re nata , meaning "as needed") offers a flexible, per diem work schedule. As a PRN therapist with Sobe Rehab, you'll receive patient assignments based on referral volume and your availability, with the freedom to accept or decline cases.
