Community Resource Navigator
Nationwide Children's Hospital Inc
Nationwide Children's Hospital is seeking a Community Health Worker at our Main Campus in Columbus, OH. You will collaborate with clinical staff to connect patients and families with social determinants of health resources and provide health education. The role requires a high school diploma, Ohio CHW certification (or completion within one year), a valid driver's license, and the ability to travel.
