If you are ready to be part of an extraordinary team of caregivers, then come work where you matter.ChaplainPosition OverviewThe Hospice Chaplain provides emotional, practical and/or spiritual guidance for patients and their families, meeting them where they are on their hospice journey. As part of the patient care team, chaplains support the unique needs of each patient, including visiting with patients and their families, coordinating with the clinical team and connecting them with community resources and services. In the end-of-life planning have the privilege of is responsible for the provision of spiritual care services to patients and families/caregivers of St. Croix Hospice, either directly or through coordination of care with other spiritual counselors.Essential Functions and SkillsConduct spiritual assessments and develop individualized care plans.Provides direct spiritual support to patients and families.Coordinates with community chaplains and spiritual counselors.Maintains records for quality assurance and compliance.Attends interdisciplinary team meetings and provides spiritual care consultation.Recruits and educates community chaplains, counselors, and volunteers.Assists in developing and maintaining a bereavement program, ensuring follow-up care.Oversees condolence calls and support group scheduling.Conducts funeral/memorial services as requested.Organizes periodic memorial services for staff, volunteers, and clergy.Supervises spiritual care volunteers as needed.Participate in quality assessment and performance improvement activities.Ensures HIPAA compliance and confidentiality of patient information.Performs other duties as assigned.QualificationsRequirements/QualificationsDemonstrated theological coursework required.If no degree, 5+ years of experience in a formal ministerial setting required.Ability to pass background, consumer report and drug test.Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.PreferredBachelor's degree in biblical studies, theology, or religious studies or equivalent theology degree preferred.Graduate of accredited seminary or school of theology (MDIV, MTS, ThM or equivalent theological degree) preferred.One or more unit(s) of Clinical Pastoral Education preferred.Physical Requirements The physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position. Reasonable accommodation may be made.Pushing/Pulling and Lifting/carrying up to 12 pounds. Occasionally up to 20pounds.Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
