POST-ACUTE CARE PHYSICIAN – ROANOKE RAPIDS, NC

The practice is actively interviewing Board-Certified or Board-Eligible physicians trained in Internal Medicine, Family Medicine, or Geriatric Medicine.

Primary Responsibilities

• Provide medical care to patients in skilled nursing and post-acute care facilities
• Manage both acute and chronic medical conditions
• Develop individualized treatment plans and coordinate follow-up care
• Collaborate with nursing staff, facility leadership, specialists, and community providers
• Complete timely and accurate clinical documentation
