Part-Time Nurse Practitioner

Nurse Practitioner – Wound Care & Primary Care (Long-Term Care) We are seeking a dedicated and experienced Nurse Practitioner to provide wound care and primary care services to patients in a long-term care setting. This is a great opportunity for a provider who enjoys working with medically complex patients and collaborating within an interdisciplinary team. Provide comprehensive wound care and primary care services to patients in skilled nursing facilities Diagnose and treat acute and chronic conditions, with a focus on complex wound management Prescribe medications, including controlled substances in accordance with Ohio state law Order, review, and follow up on labs, diagnostics, and treatment plans Collaborate closely with nursing home staff and primary care physicians to ensure continuity of care Communicate changes in patient condition and update care plans as needed Board Certification (FNP, AGNP, or related specialty) Clinical background in adult, family, or geriatric care Wound Care Experience Comprehensive patient assessment and diagnosis Chronic disease management Acute care evaluation and treatment Medication management and prescribing 3 days per week (Between Monday–Friday, 8am-5pm) 20–24 hours per week (may start off lower, around 12 hours per week and then increase after 30 days) Flexible scheduling (set your own consistent weekly schedule) If you’re passionate about delivering high-quality care to a vulnerable patient population and have strong wound care expertise, we’d love to connect with you! #J-18808-Ljbffr
