Your work will help patients navigate the healthcare system and achieve better long‑term outcomes. Key responsibilities include: Coordinating and monitoring patient care to optimize health outcomes Identifying and supporting high‑risk patients Developing and managing individualized, patient‑centered care plans Providing patient education and motivational interviewing to promote self‑management Collaborating with primary care providers, specialists, hospitals, and community resources Conducting post‑hospital follow‑ups, medication reconciliation, and care transitions Managing a caseload (up to 250 patients) with a focus on prevention and continuity of care Maintaining accurate documentation and supporting care management billing Clinic/office-based setting Occasional travel between clinic locations Physical requirements include standing, walking, and lifting up to 25 lbs Non‑exempt position; occasional hours over 40/week may be required Upper Great Lakes Family Health Center is committed to providing high‑quality, compassionate care to the communities we serve.
