Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (preferred)

Roles & Responsibilities
- Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, nutrition, safety, and self-care practices.
- Supervise and provide clinical direction to healthcare assistants to ensure quality service delivery.
- Monitor compliance with third-party payer requirements and perform on-call responsibilities as assigned.
- Maintain up-to-date patient records with accurate documentation of visits and care provided.

- Must meet mandatory continuing education requirements.
- Ability to work flexible hours and travel locally.
