Case ManagerWe are seeking a dedicated and experienced Case Manager to join our team. This position requires a strong background in acute care and will involve working with patients to ensure they receive comprehensive care throughout their treatment journey.Job ResponsibilitiesManage patient cases from admission to discharge.Coordinate with healthcare providers to develop and implement care plans.Conduct assessments and evaluate patients' needs.Work collaboratively with multidisciplinary teams to improve patient outcomes.Maintain accurate and timely documentation in Epic.Working ConditionsDay shift: 5x8 hours (08:00 - 16:30).Required to work every other weekend.This role may involve floating between similar units based on patient care needs.Local candidates are encouraged to apply.QualificationsMinimum of 2 years of acute care experience required.First-time travelers accepted provided they meet screening approval.Excellent communication and organizational skills.Additional InformationThis is an exciting opportunity for nature lovers!
